Improper injection of fat dissolving Kybella

ATX-101(Kybella) ®/ Kythera is a deoxycholic acid injection produced by Allergan, which is used to treat chin fat accumulation. It will lead to lipolysis when injected subcutaneously, and was approved for marketing by the US FDA in 2015.

The more common side effects (AES) of deoxycholic acid Kybella injection include pain, swelling, bruising, numbness, erythema and induration. These events are usually moderate and temporary.

Long term side effects AES were not mentioned in the previous literature. Today, we will learn about AEs after long-term injection of deoxycholic acid Kybella. The first case is epidermal hypertrophic scar, and the second case is long-term concave scar.

A 45 year old female patient received double chin injection of deoxycholic acid Kybella in an external plastic surgery clinic. Central anterior neck wound appeared after the second injection. After the injection, the patient immediately experienced severe pain and discomfort. A few days after the injection, the patient reported that the injection site was red, swollen and induration increased, and then the wound and dandruff appeared. The physical examination showed that there were purplish red nodules 2 cm deep in the front neck, scabs in the middle, and moderate induration around (see the following figure).

Wounds were treated with hydrosol dressing. Three weeks later, the dandruff softened and fell off, exposing a thick pink scar (see the following figure). After two months of topical steroids, triamcinolone acetonide 0 injected 0.2 mL (20 mg/mL) of hypertrophic scar, and then treated with pulsed dye laser, Meigeguoming X improved. She continued to be followed up for treatment of hypertrophic scars.

A 30-year-old woman complained of too much fat on her chin. After discussing the treatment plan with the doctor, it was decided to use deoxycholic acid Kybella in the doctor's list for treatment. The dosage of deoxycholic acid was adjusted to 2mg/cm, and deoxycholic acid was injected into the subcutaneous adipose tissue at the lower part of the chin. The doctor injects according to the method recommended by the manufacturer.

The total injection dose of deoxycholic acid Kybella is 10 mg. After 10 weeks, the patient received a second treatment and used the same technique and total dose. The patient received both treatments effortlessly.

One month after the second injection, the patient's boyfriend noticed that the patient's front neck had many sunken scars when the patient's neck was stretched excessively (see the following figure). These findings were located as the injection site of deoxycholic acid Kybella.

The patient denied any pain or discomfort in the area. The sunken scar is long-term, about 2. The scar has not disappeared with the passage of time in five years.

Possible mechanism

In pre marketing clinical trials, most of the adverse reactions of deoxycholic acid Kybella were limited to the injection site, mild to moderate and temporary. The common side effects are pain, swelling, bruising, numbness, erythema and induration. These adverse reactions are usually related to injection procedures and can be resolved in different treatment rooms. Serious adverse reactions are relatively rare.

Patient 1 formed a wound after the skin ulcer, followed by a hypertrophic scar, resulting in long-term damage to the chin skin. Superficial skin ulcer may be caused by improper injection technology, and the importance of injecting deoxycholic acid Kybella into SMF to avoid damage to high-quality supplies is emphasized. The lipolysis caused by local inflammatory reaction (responsible for removing cell fragments and rupture products) may also lead to the observed adverse events.

After the second treatment, the patient's neck showed dimple like scars, which can only be noticed when the neck is excessively expanded. The inflammatory reaction of deoxycholic acid Kybella after injection may lead to a certain degree of fibrosis, in the area X where deoxycholic acid concentration is high at the injection site.

In addition, although clinicians follow the techniques recommended by the manufacturer and the trainer used by the manufacturer, too superficial injection techniques may be the cause of dimple scars. These cases show that the clinician is not only in the neutral position, but also in the neck extension position to evaluate the patient. It is important to tell the patient about the potential risk of this sunken scar.

What is injection lipolysis?

Injection lipolysis is a new "injection fat reduction" and a new non-surgical weight loss method. Adipocytes are gradually dissolved and eliminated, acting on subcutaneous adipose tissue and microcirculation system. Light acupuncture can immediately accelerate fat metabolism, degrade and absorb fat, tighten and lift skin. And it does not affect the normal work and life, so that every beautiful woman can have a beautiful curve and a devil like figure.

Injecting fat dissolving agent can also make up for small defects on the face. In fact, it can go from the eyelids to the abdomen. Fat decomposition needle accelerates fat metabolism, degrades and absorbs fat, tightens and improves skin.
